Online Presence Relaunch and Fillable PDF Intake Forms

The goal of this project was to bring a client back online. In 2020, Community Pathways Unlimited found itself without a website. They had been hacked and taken offline due to a security issue. Their CEO was not sure how to proceed, and reached out to us to help her secure the domain and recreate a website. We first transferred the domain to our secure hosting space, then rebuilt the site. We utilized their existing logo as an anchor point along with hard-copy materials that were mailed to us about their services. After the new secure website was live, we created a Facebook page for this client with a header to match their website presence. We also digitized all of their intake paperwork to make things easier for the team in the era of Covid-19. We created a password protected portal page for the behavioral therapy providers to access all of the fillable PDF forms by intake type.
